Alternatively please follow the link below to gain access to the calendars for each clinician to schedule an intake. Please provide information about the reason for requesting an appointment and your insurance provider in the message box at the end of the request form.

Once you schedule an intake the assigned clinician will share intake documents, to include insurance information through our portal for you to complete prior to your session. Please be aware that our medical biller needs 3 business days to contact your insurance provider for information on copays/sessions coverage.
